He doesn't like to deal with petty, small matters (like asking the patient questions) & he has his team to do that for him. He doesn't like doing his clinic duty because he thinks the patients are idiots. Personally, I don't think a doctor like House would exist in the real world (his methods are always unethical), but it seems acceptable in the TV show because he is a really brilliant doctor and (like it or not) has save many lives.
